3 months pregnant!! need medicaid answers...?

\i applied for medicaid about a month ago...... my case worker asked me some questions prating to how long i wanted medical coverage for my baby. then she told me i had to fill out a non-custodial parent form... what does that form mean? do i have to fill it out? im not seeking child support or any other services i just want medicaid for the child's birth.
